1. Passengers must complete the \'Online Contact Information\' form within 96 hours of departure. The form can be found at https://tinyurl.com/zacx95et
2. Passengers must have a COVID-19 vaccination certificate showing that they were fully vaccinated at least 14 days before arrival. Vaccines accepted are Astrazeneca (SK Bioscience), Astrazeneca (Vaxveria), Covaxin, Covishield, Janssen, Moderna (Spikevax), Nuvaxovid (Novavax), Pfizer-BioNTech (Comirnaty), Sinopharm (BIBP) and Sinovac.
- This does not apply to:
- Cook Islanders. and permanent residents of Cook Isl.;
- passengers younger than 16 years.
